Helminthiases or helminthic infestations are a group of diseases caused by parasitic worms that live in the human body. According to the latest data, 1 person in 100 is a hidden carrier, and about 80% of new cases of the disease occur in children and adolescents. At the same time, helminthiases are recorded not only in underdeveloped countries: often outbreaks of enterobiasis and ascariasis occur among wealthy segments of the population.
The widespread occurrence of helminthiasis is explained by the asymptomatic course of the disease in the early stages. Also, the symptoms of invasion do not appear with a small amount of parasites in the body. For a long time, the patient may be unaware of the presence of parasites, but at the same time infect others.
Why are worms dangerous?
To date, 360 types of worms are known that are dangerous to humans. Most of them parasitize in the human intestine or liver, but, depending on the type of parasite and its life cycle (egg, larva, adult), any organs and systems can be affected: lungs, heart, central nervous system, muscles, eyeballs, and so on. - roundworms are roundworms that affect the small intestine. During the development cycle, they can migrate along with the flow of blood and lymph, enter the lungs, heart;
- whipworm is a round worm that lives in the large intestine. It is introduced into the mucous membrane with the help of a thin head, damaging the integrity of tissues and causing micronutrients;
- opisthorchis are flat worms that affect the liver. Parasites clog the ducts, causing bile stagnation and disrupting the normal functioning of the organ;
- Krivogolovki - roundworms parasitizing in the duodenum. Infection occurs through contact with larvae, which pierce the skin, enter the bloodstream and spread throughout the body;
- bovine tapeworm - tapeworm, reaching a length of 10 meters. Occupies almost the entire intestine, leading to severe exhaustion;
- intestinal acne - roundworms that invade the colon and cecum. To lay eggs, the worms move closer to the anus, causing severe itching in the anal area;
- echinococcus - tapeworms that affect not only the gastrointestinal tract, but also the liver and lungs. They cause echinococcosis, a disease in which cysts form in various organs in a person.
How are worms transmitted?
According to the WHO, more than 1.5 billion people worldwide suffer from soil-borne helminthiasis. There is a widespread misconception that it is possible to become infected with worms only by consuming low-quality products: untreated water, unwashed fruits and vegetables, or unprocessed meat and fish. But this is not the case. Some of the parasites can penetrate through intact skin.
The larvae of helminths penetrate the skin, enter the capillaries, and are carried throughout the body along with the blood stream. For a person, such punctures are not sensitive - it is quite difficult to suspect an infection. You can also become infected with parasites by inhaling microscopic eggs along with dust.
How to understand if there are worms?
Depending on which organs have suffered from helminthic invasion, patients may develop conjunctivitis, cough, headaches, edema, tremors of the extremities. In the case of severe vitamin deficiency, pain appears in the joints, hair falls out, nails begin to exfoliate. In children, helminthiasis can lead to a delay in physical and intellectual development.
Common symptoms of worms in humans are:
- malaise, weakness, fatigue;
- allergies such as skin rash, cough, asthma attacks;
- decreased or increased appetite;
- nausea, vomiting without poisoning;
- stomach ache;
- diarrhea or constipation;
- weight loss, even if the appetite is good;
- sleep problems, insomnia;
- inflamed or enlarged lymph nodes;
- temperature rise for no reason;
- pain in muscles and joints without stress;
- snoring or teeth grinding during sleep;
- the presence of worms in the feces;
- itching in the anal area (often with pinworms).
How do you know if there are worms?
It is impossible to independently determine the presence of helminthic invasion. In the initial stages, the disease can be almost asymptomatic. The patient does not experience pain, immunity for some time can suppress the pathogenic effect of toxins and allergens. As a rule, exacerbation begins during the migration of larvae or with an increase in the number of worms. The stronger the invasion (i.e. the more parasites), the more symptoms appear.
However, the asymptomatic course of the invasion is dangerous - the patient infects others, and his state of health is gradually deteriorating. To identify the disease, it is necessary to periodically undergo a preventive examination in the hospital. As part of prevention, the therapist prescribes tests for worms at least once a year. Analysis for helminths
To identify helminthic invasion, the following tests can be prescribed:
- stool analysis. Feces are examined for the presence of helminth eggs. The analysis does not require preliminary preparation: it is enough for the patient to bring a sample of feces;
- analysis for enterobiasis. To determine enterobiasis (a disease caused by pinworms), it is enough to take a scraping (smear) from the anal area. Usually, the analysis is prescribed in the morning before going to the toilet;
- blood test for specific antibodies. The most revealing study to identify such parasites as roundworms, opisthorchis, eels, lamblia.
How much is a feces test for worms valid?
Stool analysis for worms allows specialists to detect helminth eggs (if any). This study does not give a 100% guarantee of detection, since the number of eggs depends on the life cycle of the worms and from time to time they may not be detected in the feces, although the invasion persists. Therefore, in most cases, doctors recommend taking a stool test two or three times with a short interval between examinations.
Blood test for parasites
A blood test for specific antibodies is the most informative method for detecting helminthic invasion. After the analysis, the doctor can not only talk about the presence of worms, but also establish their appearance, which means that he selects the most effective therapeutic complex.

Is it necessary to "chase worms" for prevention
- When I was studying at a medical school, we were told that in Belarus one hundred percent defeat of the population by helminths. Because of this, as well as on the recommendation of the pediatrician, while the children were small, we "drove" worms with the whole family twice a year. Now the children have grown up, they observe hygiene, and I came across information that the situation with helminthiasis in the country has become better. Do I need to continue to "chase worms" twice a year? A cat lives at home (we do this for him regularly).
- Indeed, 20-30 years ago in Belarus, almost 100% of the population suffered helminthiases one or several times, mainly roundworms and pinworms prevailed. This was played by an imperfect water supply system, non-compliance with sanitary standards, including in children's groups (kindergartens and schools). But today the situation has changed significantly. The number of these diseases has decreased significantly. But I want to draw your attention to zooanthroponous diseases, in particular toxocariasis. Today, there are more dogs in the cities (Toxocar reservoir). People get infected through their feces. Toxocars can penetrate into any human organ: eyes, lungs, brain, gastrointestinal tract. Currently, there are many requests on this matter. Therefore, you cannot walk dogs in public places, in playgrounds, you should clean up feces after them, as is customary in all civilized countries. Toxocariasis is difficult to diagnose.
Animals can be treated prophylactically without diagnosis. There are complex preparations, three in one (albendazole, praziquantel, ivermectin - for the prevention of dirofilariasis in dogs). They can be given to animals once a year.
But people are not recommended to take such drugs prophylactically, without diagnosis and a doctor's prescription, especially for pregnant women and young children!
Most helminths are intestinal. Therefore, the main diagnostic method is the analysis of feces for eggs of worms. Antibodies are a suggestive analysis, it is important for tissue helminthiasis (echinococcosis, toxocariasis, trichinosis). Cats are a reservoir of toxoplasmosis.
- What drugs can be taken for prophylaxis against parasites for adults and children without a doctor's prescription?
- All drugs are toxic. Therefore, without appointing a doctor for the prevention, they cannot be taken, especially pregnant and children.
- I have a dog in my house, I give her anthelmintic drugs periodically. How often take me yourself? What drugs are most effective?
- Dogs can be, especially if they noticed worms in Kale. But you yourself should not do this without a diagnosis.

- Helminths, in addition to allergic, toxic and depleting effects, also have oncogenic effects. But, as a rule, this effect is very delayed. Parasites cause mutations in chromosomes, including oncological ones. This can lead in the long term to the appearance of neoplasms, including leukemia. By the time of chemotherapy, they may no longer be there. Since these parasites do not live long.

- If the dermatitis is seborrheic, then it is unlikely that it is a helminthic invasion. You can take an analysis of feces for eggs of worms. But the drug should not be taken until the results are obtained. You can also get tested for HIV infection, since dermatitis can be one of its manifestations.
- How do infectious disease doctors understand that this is “their” patient, and not the symptomatology of some other disease?
- Clinical experience helps us in this. This is a complex diagnostic process. A large number of laboratory tests, especially for antibodies, cause unnecessary trouble for patients if they are not explained by a doctor. Therefore, the doctor must interpret them. And appoint too.
How and to whom to treat toxocariasis and ascariasis
- I am 24 years old. About 6 years ago, the nose began to bother me (congestion, snot). They set up sinusitis, prescribed antibiotics. Did not help. - Since 2016, almost every year I have been suffering from allergic reactions up to urticaria. Twice I was in the hospital with her. In 2018, he underwent a comprehensive examination by an allergist and other specialists, but the allergen was never identified. In the same year he passed an analysis for antibodies to toxocars. As a result, toxocariasis was revealed. At the end of 2018, he drank a course of albendazole (2 weeks, 2 tablets a day, then a 10-day break, then another 2 weeks, 2 tablets). But this year, a blood test showed toxocariasis again. Can toxocars cause acute allergic reactions? What is the best way to recover from them?
- Toxocara can cause allergic reactions. Toxocars died long ago after treatment in 2018. The fact that antibodies were detected this year may indicate a past disease, but does not mean a new infection. The whole picture should be analyzed with an infectious disease doctor, it's hard to say in absentia.
- The analysis showed toxocariasis. Can Nemosol (400 mg) be treated?
- The presence of antibodies does not indicate a disease at the moment. It is necessary to analyze the whole situation as a whole. If the diagnosis is confirmed, then this drug can be treated under the supervision of an infectious disease doctor.
- My brother has lamblia. He treated 5 times. But he cannot get rid of. What to do?
- If the disease was treated correctly, then Giardia should not be. But the antibodies remained for life. To confirm the diagnosis, you need to pass an analysis of feces for lamblia cysts. If they are found, you need to be treated again, since re-infection is quite possible.
- How to get rid of toxocara and roundworm? There is no infectious disease doctor in the district. The therapist prescribed nemosol 400 mg once, repeat after 2 weeks. Treatment is ineffective.
- The presence of antibodies does not mean that there is an infection, since antibodies persist for life. When the parasite dies, the amount of antibodies increases. And that shouldn't scare you in your analyzes.
- If pinworms are found during pregnancy, is there a danger to the fetus, the health of the expectant mother? And is it necessary to treat?
- For the fetus and mother, pinworms are not dangerous. Anthelmintic drugs are undesirable during pregnancy. It is necessary to observe hygiene so that there is no autoinfection: change your underwear several times a day, wash your hands so that eggs do not fall back into your mouth. If the pinworms come out of the anus, they will not get back to us.
- Is it possible intrauterine infection of a child with ascariasis from an infected mother? The child scratches hands and feet at night, has skin rashes, itching of the skin from birth. The analyzes revealed ascariasis. The child is 3 years old, the older one is 5 years old (both have ascariasis and similar symptoms). We were treated with pyrantel, but after a while the symptoms returned. How to cure ascariasis?
- Intrauterine infection of a child with ascariasis from an infected mother is impossible. Pirantel is an effective remedy for roundworms. If eggs are found in the feces, then they should be treated, and if antibodies are found in the blood test, then this does not mean the disease at the present time.

Causes of worm infection in children

Worm infestations are more common in children than in adults. Worms in children under one year old are not as common as in children 1.5 - 3 years old, who actively explore the world, taste various objects and become infected much more often.
Helminthiases are dangerous at any age. Worms in a one-year-old child are a special case, because the baby's body has not yet matured, the immune system is weak, and the presence of worms in children can cause frequent acute respiratory diseases, a tendency to allergies, and diseases of internal organs.
The presence of worms in babies is a big health problem, since the worms feed on the substances required for the growth and full development of the baby.
Symptoms of worms in children
Signs of worms in children depend on the habitat and activity of the worms in the child's body. The most common worms - roundworms in babies, pinworms, tapeworms - live in the intestines, but some of certain parasites can move from organ to organ. For example, roundworms first enter the stomach, and from it with blood they enter the lungs or liver. There they grow for three weeks, then re-enter the intestines or stomach.
Pinworms often inhabit the anus, causing severe itching. In girls, they can be found in the genitals, causing vulvovaginitis.
Opisthorchis parasites often live in the liver and pancreas. Echinococcus can affect muscles and bones, tapeworms even penetrate the brain. Also, some types of helminths are found in the respiratory organs, causing a persistent cough.
How to understand that a child has worms?
According to the location of the worms in children, there may be the following signs of the presence of worms in a child:
- Constipation. Large worms can block the lumen of the intestine, which causes impaired stool flow. Constipation may last for more than 2 days.
- Diarrhea. Some parasites secrete a special substance that stimulates the body to excrete undigested food particles and large amounts of water.
- Irritable Bowel Syndrome. Helminths can cause inflammation in the intestines. This causes malabsorption of nutrients and irregular bowel movements.
- Flatulence. Some types of helminths provoke fermentation processes in the small intestine, this causes excessive gas formation. If you don't get rid of the worms in time, flatulence can last for months.
- Soreness in joints, muscles. This happens when parasites invade a joint or into the thickness of muscle tissue and destroy it.
- Allergy. The waste products of worms can activate special cells in the body that are responsible for the occurrence of allergic reactions.
- Worms can cause skin diseases. Acne, urticaria, papillomas and diathesis appear. Sometimes infection with worms leads to increased fragility of hair and nails.
- Anemia. Some types of worms are able to adhere to the intestinal walls and receive not only nutrients from them, but also blood. Because of this, the child develops anemia, which is diagnosed with a clinical blood test.
- Change in body weight. As a rule, as a result of infection with worms, body weight is sharply reduced, since the worms absorb more of the nutrients that enter the body with food. Having a helminthic infection often causes loss of appetite due to the ingress of toxins into the bloodstream. But often the child experiences an increase in body weight - this is a defensive reaction to the spread of worms.
- Disorders of the nervous system. The child has a moody and irritable behavior. Depression may develop in schoolchildren and adolescents against the background of helminthiasis.
- Sleep disturbances. When a child wakes up every night, this may indicate an infection process. At night, some types of worms leave the body through the anus, which is accompanied by severe itching, which prevents the baby from sleeping.
- Jaundice. More common with tapeworm infections.
- Chronic fatigue syndrome. The child is constantly weak and tired quickly due to the lack of nutrients in the body. Memory problems develop, emotional stress and drowsiness develop.
- Immunity disorders. Due to helminthiasis, a child is much more likely to develop diseases of a viral genesis, an allergic reaction to ordinary foods appears. In the intestine, dysbiosis, colitis often develop, on the face - rashes, diathesis and herpes.
